Federal regulations give patients and providers rights in relation to patient healthcare information. Among those rights are the right of patients to see their healthcare information maintained by EVA Care during and after their treatment, the right of patients to receive a copy of their information maintained by EVA Care during and after their treatment, and to have EVA Care correct their information if errors are found. Additionally, EVA Care has the right to share patients' healthcare information for the purposes of insurance reimbursement as well as for treatment. EVA Care will notify its patients of its intent to share their healthcare information for any such purposes prior to commencement of first treatment. Patients not using insurance coverage for services will have information shared only for treatment purposes.

EVA Care maintains the confidentiality of patients' care and health information. EVA Care shares information only for the purposes of insurance reimbursement and treatment in accordance with HIPAA regulations. EVA Care does not share patient care and health information for purposes of marketing.
Exceptions to confidentiality occur when healthcare providers at EVA Care must act to protect patients and other persons from harm or to answer court orders for treatment information. To protect patients and others from harm, healthcare providers at EVA Care will notify appropriate authorities in accordance with the law.

Patients may request that information about their treatment be shared by waiving confidentiality in writing. Waiving confidentiality may include requests to send records to other providers or discussion of treatment with others such as relatives, school officials, attorneys, or employers.
